Private hospital nurse pay: what PSA's survey shows

The Philippine Statistics Authority's PSA 2024 Occupational Wages Survey is the official source for private sector pay by occupation. It covers formal private establishments with 10 or more workers and measures the average monthly wage rate (basic pay plus regular, guaranteed allowances) of full-time workers in the payroll period that included 31 August 2024. It does not cover public hospitals or health services. For health, the occupation figures are for the industry "Human Health Activities except Public Health Activities": private hospitals, clinics and similar establishments.

Public vs private: an honest comparison

To compare like with like in time, set the August 2024 survey against the 2024 schedule (first tranche of EO 64, in effect from 1 January 2024). A Nurse I at step 1 was then paid ₱38,413 a month in basic salary: 58% more than the private average of ₱24,343, and 44% more than the top of PSA's 90% range (₱26,612). Government pay also keeps rising each January until 2027.

Four things narrow or complicate that gap:

  • Different measures. The private figure is an average across all experience levels and includes regular allowances. The public figure is the entry rate, basic salary only, before PERA and other allowances. Neither includes the 13th month pay or year-end bonus.
  • Who gets the government rate. The schedule covers regular, casual and contractual government personnel. People hired through job orders and contracts of service are not covered (NBC 601, item 4.4.4), so a nurse working in a public hospital on such a contract is not paid these rates.
  • LGU hospitals. A provincial, city or municipal hospital may pay a share of the schedule set by the LGU's income class, down to 65% in a 6th class municipality (EO 64, Section 4). Nurse I at 80% is ₱33,742 a month in 2026.
  • Survey limits. Small clinics with fewer than 10 workers are outside the survey, and the figure is a sample estimate: PSA's 90% range for private nurses runs from ₱22,075 to ₱26,612.

Midwives, nursing attendants and medical officers

The same index sets the salary grades of the other health positions. Some of them, from DBM's Index of Occupational Services (CY 2022), with the 2026 rates:

Medical Officer IV 23 ₱91,306 ₱101,318
Medical Officer V 25 ₱116,643 ₱130,073

Monthly, national government rates. Medical Officer I to V are at salary grades 16, 18, 21, 23, 25. Private comparison (August 2024, PSA): midwifery professionals ₱16,306, health care assistants ₱16,687, generalist medical practitioners ₱47,945.

How a government nurse's pay is set

Every government position is classified into one of 33 salary grades. DBM's Index of Occupational Services, issued by Budget Circular No. 2022-2, lists the grade of each position title, and NBC 601 adjusts salaries provided the positions and grades follow that index. The nurse series runs Nurse I SG 15, Nurse II SG 16, Nurse III SG 17, Nurse IV SG 19, Nurse V SG 20, Nurse VI SG 22, Nurse VII SG 24: the jumps from Nurse III to IV, Nurse V to VI and Nurse VI to VII are two grades each.

New hires start at step 1 (NBC 601, item 5.1.4). A nurse then moves up a step for every three years of continuous satisfactory service in the same position, and agencies can grant a step for meritorious performance, under CSC-DBM Joint Circular No. 1, s. 2012. Moving to a higher Nurse level is a promotion to a different position, which usually raises pay more than a step does.

The index covers national government agencies and certain GOCCs. Provinces, cities and municipalities classify their positions under a separate index (DBM Local Budget Circular No. 137, 13 July 2021), so an LGU nurse's grade may differ, and LGUs pay the share of the schedule set for their income class (EO 64, Section 4).

Limits

  • Government figures are basic salary from the national schedule. PERA, hazard pay, allowances, the mid-year and year-end bonuses, the cash gift and other benefits are set by other laws and issuances and are not included.
  • Take-home is an estimate for a regular employee: GSIS at 9% of basic salary, PhilHealth at 2.5% (5% shared) up to ₱100,000, Pag-IBIG at 2% of up to ₱10,000, and withholding tax worked out on the year. Loans and other deductions are not taken off.
  • Salary grades are from the national IOS (CY 2022). LGU positions follow their own index and LGU pay may be a percentage of the schedule; your appointment and notice of salary adjustment show your grade and rate.
  • Private pay is from August 2024, the latest survey. PSA runs it every two years. It is an average wage rate, not a median, and covers establishments with 10 or more workers only.
  • Private figures by occupation are for one industry (private human health activities). Nurses working for companies, schools or agencies outside that industry are not in that figure.
  • The 2027 rates are those set in Executive Order No. 64; DBM normally confirms each tranche in a National Budget Circular in January.
